Alignment, Agency and Autonomy in Frontier AI: A Systems Engineering Perspective
As artificial intelligence scales, the concepts of alignment, agency, and autonomy have become central to AI safety, governance, and control. However, even in human contexts, these terms lack universal definitions, varying across disciplines such as philosophy, psychology, law, computer science, mathematics, and political science. This inconsistency complicates their application to AI, where differing interpretations lead to conflicting approaches in system design and regulation. This paper traces the historical, philosophical, and technical evolution of these concepts, emphasizing how their definitions influence AI development, deployment, and oversight. We argue that the urgency surrounding AI alignment and autonomy stems not only from technical advancements but also from the increasing deployment of AI in high-stakes decision making. Using Agentic AI as a case study, we examine the emergent properties of machine agency and autonomy, highlighting the risks of misalignment in real-world systems. Through an analysis of automation failures (Tesla Autopilot, Boeing 737 MAX), multi-agent coordination (Metas CICERO), and evolving AI architectures (DeepMinds AlphaZero, OpenAIs AutoGPT), we assess the governance and safety challenges posed by frontier AI.

Tesla Autopilot Was Uniquely Risky--and May Still Be
A federal report published today found that Tesla's Autopilot system was involved in at least 13 fatal crashes in which drivers misused the system in ways the automaker should have foreseen--and done more to prevent. Not only that, but the report called out Tesla as an "industry outlier" because its driver assistance features lacked some of the basic precautions taken by its competitors. Now regulators are questioning whether a Tesla Autopilot update designed to fix these basic design issues and prevent fatal incidents has gone far enough. These fatal crashes killed 14 people and injured 49, according to data collected and published by the National Highway Traffic Safety Administration, the federal road-safety regulator in the US. At least half of the 109 "frontal plane" crashes closely examined by government engineers--those in which a Tesla crashed into a vehicle or obstacle directly in its path--involved hazards visible five seconds or more before impact.

Tesla Autopilot: Explained
As Tesla's "Full-Self Driving Capability" begins to roll out, many of its owners are considering spending the $10, 100 to upgrade, however many of its buyers aren't even aware of the differences between the three levels of self driving; autopilot, enhanced autopilot and full self-driving. So this is for all of you out there who have no idea what the differences are and can't be bothered to pay a visit to Tesla's website. Tesla cars come standard with the "Future of Driving" which includes eight cameras providing 360 of visibility surrounding the car for maximum protection. They also come stock with air bags, brake assist, electronic stability control, daytime running lights, child safety locks and traction control. Teslas are largely known for their acclaimed "full-self driving".
US Expands Safety Probe Into Tesla Autopilot
US regulators expanded a probe into Tesla's "Autopilot" system, moving the investigation closer to a potential recall of a controversial feature in Elon Musk's electric vehicles. The National Highway Traffic Safety Administration is investigating whether "Autopilot and associated Tesla systems may exacerbate human factors or behavioral safety risks by undermining the effectiveness of the driver's supervision," according to a summary statement. The agency now considers the probe an "engineering analysis" -- which in NHTSA parlance upgrades the status from a "preliminary evaluation" -- to determine "whether a safety recall should be initiated or the investigation should be closed." Tesla did not immediately respond to a request for comment. NHTSA opened the probe in August 2021 after identifying 11 crashes involving a first responder vehicle and a Tesla in which Autopilot or Traffic Aware Cruise Control was engaged, and five additional cases were later found that fit into this group.

Tesla Autopilot under investigation following crash that killed three people
A recent Model S crash that killed three people has sparked another Federal probe into Tesla's Autopilot system, The Wall Street Journal has reported. The National Highway Traffic Safety Administration (NHTSA) is conducting the investigation and said it's currently looking into more than 30 incidents involving Tesla's Autopilot. The accident occurred on May 12th in Newport Beach's Mariners Mile strip, according to the Orange County Register. The EV reportedly struck a curb and ran into construction equipment, killing all three occupants. Three construction workers were also sent to hospital with non-life-threatening injuries.

Tesla Autopilot's Role in Deadly Vehicle Crash Is Probed by Safety Regulators
U.S. auto-safety regulators have opened a special crash investigation into a fatal wreck involving a Tesla Inc. vehicle that has left three people dead. The National Highway Traffic Safety Administration disclosed the probe Wednesday, identifying the vehicle as a 2022 Tesla Model S. It added the incident to a list of auto crashes it is investigating that are potentially linked to semiautonomous driving features.
Tesla Model 3 on Autopilot hits Highway Patrol vehicle in Florida
A Tesla Model 3 struck a Florida Highway Patrol (FHP) trooper's vehicle on Saturday just weeks after the NHTSA established an investigation into the semi-autonomous driving functionality. The driver and FHP confirmed the vehicle was operating on Autopilot. According to the Orlando Sentinel, a 27-year old was driving his Model 3 westbound on Interstate 4 near Orlando at around 5 a.m. The driver stated that the vehicle was operating on Autopilot, according to FHP and ABC affiliate WFTV9. The driver of the Tesla, along with the owner of the disabled vehicle, had minor injuries.

The agency "is committed to ensuring the highest standards of safety on the nation's roadways," a spokesperson said, and in order to "better understand the causes of certain Tesla crashes, NHTSA is opening a preliminary evaluation into Tesla Autopilot systems." Tesla founder Elon Musk has defended the Autopilot system and the electric automaker warns that it requires "active driver supervision" behind the wheel. But critics, including in Congress, say the system can be easily fooled and that its name gives drivers a false sense of confidence.

The incidents dating back to 2018 included one fatal crash and seven that resulted in injuries, according to the National Highway Traffic Safety Administration. Tesla founder Elon Musk has defended the Autopilot system and the electric automaker warns that it requires "active driver supervision" behind the wheel, but critics, including in Congress, say the system can be easily fooled and have called for NHTSA to take action. Testers with the magazine Consumer Reports demonstrated in a video that Autopilot could be fooled into driving with nobody behind the wheel, an exercise also shown in widely-seen videos on Tik-Tok and other social media platforms. "A preliminary evaluation starts the agency's fact-finding mission and allows additional information and data to be collected," a NHTSA spokesperson said.

National Highway Traffic Safety Administration launches investigation into Tesla Autopilot over emergency responder crashes
U.S. auto-safety regulators have launched an investigation into Tesla's partially self-driving car system after nearly a dozen reports of the company's vehicles crashing into cars at the scenes of incidents involving emergency responders. The National Highway Traffic Safety Administration on Friday opened the probe into Tesla's Autopilot, which automatically steers, brakes and accelerates the vehicle on most roads with lanes. While the system can drive the vehicle on its own in many circumstances, drivers are supposed to keep their hands on the wheel in case they need to take over when Autopilot encounters a situation that's too complex for it to handle on its own. But Autopilot has come under scrutiny on a number of occasions in recent years. The National Transportation Safety Board and NHTSA have investigated Autopilot multiple times, including for a 2016 crash that killed a man in Florida who authorities said had too much confidence in the system's capabilities.
